Loss Prevention Bulletin Issue 310

We are pleased to announce that the latest issue of the Loss Prevention Bulletin (LPB) has now been published.
Issue 310 brings together a wide-ranging collection of case studies, technical insights, and practical lessons from across the process safety community. This edition examines both long-standing hazards and emerging risks, reinforcing the importance of continued vigilance, effective risk management, and organisational learning.
The issue opens with a feature on popcorn polymer, revisiting a known but sometimes overlooked hazard that continues to present serious challenges in industrial operations. Further incident-based learning is provided through an article on a fatal tank explosion, highlighting the critical importance of understanding ignition risks, maintaining effective controls, and learning from past events.
Additional articles explore the role of human capability and workload in safe operations, as well as the reasons why process safety incidents continue to occur despite established systems and controls. The first article in this new series focuses on invisible risk, encouraging readers to consider how unseen or underestimated hazards can develop within complex operating environments.
This issue also continues the Monograph Series with Part 3, focusing on corrosion. The article reflects on how familiar degradation mechanisms can still lead to significant incidents when not properly understood, monitored, or managed. Further technical content examines hidden explosive hazards in aged 2-propanol, including the spontaneous formation of TATP during long-term storage.
The issue concludes with an article on modelling battery hazards for safer operations, reflecting the growing importance of understanding emerging energy-related risks and applying robust process safety principles to new technologies.
